Why do I need to come in for a consultation before my appointment?

At Elevation, we are dedicated to providing the safest treatments and the best results. In order to do this, our MD or NP need to complete a thorough history and physical, discuss treatment goals, and collaboratively determine a treatment plan with you. Aesthetic treatments are medical treatments that come with risks and/or side effects. All patients are entitled to informed consent and a thorough understanding of all aspects of their treatment. Some treatments also require a pre-treatment regimen to achieve optimal and safe results at the time of treatment.

Do you have a referral program?

Yes, we do! If you refer a friend and they come in for a treatment, they will receive $50 off of their treatment or product and you will get $50 off your next treatment or product.

What products do you carry?

We carry Botox, Dysport, and Jeuveau lines of neuromodulators and the Juvederm, Restylane, and Revanesse lines of dermal fillers. Please visit the individual manufacturers websites for more product and safety information.

When will I see results and how long will my botox or filler last?

Results vary by treatment. Neuromodulators (Botox, Dysport, Jeuveau) will take full effect by two weeks and typically last 3-4 months. Filler results vary upon the type of filler, but have an average of 12 months longevity.

Will I look overdone or frozen?

This is a huge concern for most patients! The answer is you do not need to look frozen or fake! When we consult with you, we listen and form a treatment plan that reflects your goals. Come visit us to receive tailored and skilled injections. We take our time with placement of your injections and this determines how much movement you have as a result. We will follow up with you and offer enhancements to your services to fine tune your treatment to your liking.

How much will it cost?

Skincare services are listed on the “Skincare” page of our website. Cost of all other treatments is dependent upon the specific treatment, series of treatments required, or desired results. Schedule a consultation with us for pricing.

Do you take insurance?

We do not take health insurance at this point in time.

Do you take Alle or Aspire points?

We do! Talk to your injector about using these when coming in for treatment.

  • To ensure that you get an appointment for the date and time you desire, we suggest booking all appointments a few weeks in advance. Call the office for the most up-to-date availability. Please note that we require a credit card to reserve your appointment and must maintain a valid credit card on file.

  • We provide consultation and in-depth skin analysis with an expert provider on our team. We believe an informed client with realistic expectations will be most satisfied with our treatments and results. Our consultations include education about your unique skin type, a comprehensive discussion about all options available to you, both immediate and long term, and a clear plan of action based on your individual goals. Initial consultations require a fee of $100. If a patient books a treatment and renders the service, the $100 consultation fee will be credited to the cost of the treatment. If a patient does not book a treatment, the consultation fee is nonrefundable.

  • As a courtesy to our providers, for non-group appointments, please provide a minimum of twenty-four (24) hours notice should you need to cancel or reschedule an appointment. You will be charged $75 if an appointment is canceled or rescheduled less than twenty-four (24) hours in advance or if there is a no-show. For group appointments, there is a seventy two (72) hour cancellation policy, and the credit card on file will be charged $200 if canceled less than seventy two (72) hours in advance. If you arrive more than ten (10) minutes late for your appointment, you may be required to reschedule to avoid disrupting other patients’ appointments.

  • Pricing for products and services is subject to change any time. Once prepaid, you have up to 1 year to redeem the purchased service(s).

    If there are any payment issues with a patient’s credit or debit card, Cherry or CareCredit –including but not limited to chargebacks, fraud issues, etc. - Elevation reserves the right to refuse future payment by credit or debit card, Cherry or CareCredit. In this case we will require payment in cash at the time of service.

    Elevation does not accept checks.

  • Skin Care Products:

    The medical grade skin care products that we offer for purchase are sensitive to handling and storage conditions. Because we cannot ensure the products purchased have been properly stored, we are unable to accept returns on skin care items.

    Gift Cards:

    We do not accept returns or exchanges on gift card purchases.

    Treatments and Services:

    Prepayments for services are refundable within ten (10) days of purchase if services have not been rendered. If a package is purchased and partially used, the remaining balance cannot be refunded. The balance may be applied to any other service or product at Elevation.

    Payment for services received can not be refunded. If you are dissatisfied with a treatment for any reason we strongly encourage you to call Elevation directly at (518) 409-4696.

    On-site Training Courses:

    For training courses, payments are non-refundable.

  • You are absolutely welcome to bring a responsible adult to accompany you as a support person during a consultation or while receiving a treatment. To ensure the safety of children, however, we ask that parents or guardians make arrangements for child care during their visit to Elevation. A parent or legal guardian must accompany and give informed consent for minor children under the age of eighteen (18) receiving treatments.

  • For the health and safety of our clients, we have a No Pets policy. Service animals are dogs of any size or breed who are trained to assist with or perform a task. Service animals are permitted during office consultations and in common areas. Service animals are prohibited in treatment rooms because their presence can fundamentally alter the sterility of the treatment room.

  • Your privacy is essential to us. The following outlines our privacy policy to help you understand how we collect, use, communicate, disclose, and use personal information.

    Before or at the time of collecting personal information, we will identify the purposes for which information is collected. We will collect and use personal information solely to fulfill our specified purposes and for other compatible purposes. We will only retain personal information as long as necessary to fulfill the specified purposes. We will collect personal information by lawful and fair means and, where appropriate, with the knowledge or consent of the individual concerned. Personal data should be relevant to the purposes for which it is to be used and, to the extent necessary for those purposes, should be accurate, complete, and up-to-date.

    We will protect personal information by reasonable security safeguards against loss or theft and unauthorized access, disclosure, copying, use, or modification. We will make readily available to customers information about our policies and practices relating to the management of personal information. We are committed to conducting our business in accordance with these principles in order to ensure that the confidentiality of personal information is protected and maintained.

    You can permanently disable your account and remove your information from our database at any time by sending an email to info@elevationqby.com. You can see what information is deleted and what we continue to store after the account is disabled in our privacy policy.

    These terms remain in effect after your account is disabled.

    We collect information automatically as you navigate the site or through our analytics providers. We may store usage information such as the type of device you use to access our Website, including IP address, device ID, the pages you visit or request, links clicked, referring sites, user interactions, and your search terms. We also derive your location from your IP address.

  • You agree to defend, indemnify and hold harmless the Company, its affiliates, subsidiaries, and each of its and their respective directors, officers, shareholders, managers, employees, agents, partners, representatives, licensors, successors, and assigns (collectively, “Related Parties”) from any claim, demand, loss, award of damages, expense, or cost (including reasonable attorneys’ fees) that arises out of:

    (a) Your use of, inability to use or access, or reliance on, the Platform or the Services, or any goods or services obtained therefrom;

    (b) Your provision or receipt of a Treatment;

    (c) Your travel in connection with the provision or receipt of a Treatment;

    (d) Your interaction with any other User;

    (e) Any claim that you violated any provision, term, condition, covenant, warranty, or representation in this Agreement;

    (f) Any violation of this Agreement by you or any other person using your User Account, whether or not such usage is expressly authorized by you;

    (g) Any harm to person or property resulting from your acts or omissions, whether such acts are intentional, negligent, or otherwise;

    (h) The Company’s use, disclosure, or preservation of User Content; or

    (i) Your violation of any rights of another, including intellectual property rights.
